When the insured passed away,¥40,000 is supplied.

[Documents required]

• National Health Insurance Certificate (of the said person)
   
• Certificate of death (cremation / burial certificate etc.)
   
• Documents to certify the person who presided the funeral (the chief mourner) (the letter of appreciation for attending the funeral service, the receipt of the funeral ceremony or the like, in which the chief mourner's name is entered )
   
• Informal seal (Mitomein) of the chief mourner
   
• Account number for money transfer in the name of the chief mourner
