Application for Seal Registration Certificate Issue

Neyagawa City (Neyagawa-shi) issues Seal Register Certificate for citizens, who have registered their seals in the city, at Citizens' Division (Shimin-ka) , each Citizen Center (Kori, Kayashima, West, East), City Hall Service Center Neyagawa-ya (Shiyakusho Service-dokoro Neyagawa-ya) and Horimizo service counter of Kayashima Citizen Center (Kayashima Shimin Center Horimizo Service Madoguchi).

◎ Requisite

Seal Registration Card
(Seal Registration Certificate will not be issued even if the said person bring his/her registered seal.JITUIN )

　　Reception place & time

 

The Department ( shimin-ka )	Monday to Friday ( Exclude holiday, the period from 29th December to 3rd January. ) and available on the 4th Sunday in every month. 9:00 a.m. to 5:30 p.m.
Each citizen center ( Kouri, Kayashimai )	Monday to Friday ( Exclude Holiday, the period from 29th December to 3rd January )   9:00 a.m. to 5:30 p.m.
Nishi and Higashi citizen center	Every day except for the period from 29th December to 3rd January.   9:00a.m. to 5;30 p.m.
Hrimizo service counter	Monday to Friday Exclude Holiday, the period from 29th December to 3rd January.   10:00 a.m. to 5:00 p.m..
Service dokoro  Neyagawa-ya, City Hall	Every day except for the period from 29th December to 3rd January.   10:00 a.m. to 8:00 p.m.

◎ How to request

Please fill out the necessary items (Such as address, full name, date of birth of the registrant of the seal) on an application form for issue of Seal Registration Certificate to hand in it with Seal Registration Card.

Without your Seal Registration Card, Seal Registration Certificate will not be Issued.
Please cancel your registration by turning  in the Seal Registration Abolishment form and go through the procedure again to renew your Seal Registration at the department ( shimin-ka ) of the Municipal Office.
